Self-Certification

I certify under penalty of perjury that:

(A) I have a hearing loss that necessitates the use of ClearCaptions captioned telephone service;

(B) I understand that the captioning on ClearCaptions captioned telephone service may be provided by a live communications assistant who listens to the other party on the line and provides the text on the captioned phone. The captioning also may employ speech recognition software alone, without a communications assistant;

(C) I understand that the cost of captioning each ClearCaptions Internet protocol captioned telephone call is funded through a federal program; and

(D) I will not permit, to the best of my ability, persons who have not registered to use ClearCaptions Internet protocol captioned telephone service to make captioned telephone calls on my registered ClearCaptions IP captioned telephone service or device.
